Before you apply
However you apply to SelectPlan for Women, you’ll want to get this information together first:
1. Proof of your pre-tax and pre-deduction income. This can include:
- A full month of pay stubs OR
- One pay stub if it is a typical pay period OR
- An award letter describing your income OR
- A current tax return OR
- An employer's letter stating your gross monthly income
- If you’re self-employed, copies of tax returns or other financial records
2. Proof of your identity and citizenship
- U.S. Birth certificate OR
- Proof of satisfactory immigration status
- If you need help gathering these items, we can help you when you apply
3. Proof that you're a Pennsylvania resident
- PA driver’s license OR
- PA state ID
